Citizenship and Emergency Contact
Please note that we ask your citizenship in order to satisfy the rules, regulations, and possible restrictions, for all participating universities and medical facilities.
When selecting an emergency contact please chose someone who you trust and feel comfortable making decision for you under extreme circumstances. Please ensure the numbers that you provide will allow us to successfully contact that person if such an incident should arise.
Activities/Honors and Awards
List all relevant activities, honors, and award but be selective. If you have more activities than can fit in the space provided do not include the ones that are not significant. Most importantly, your activities should represent your varied talents and passions outside the academic setting. The selection committee is trying to get a sense of who you are personally and professionally.
Letters of Recommendation
Letters of recommendation are extremely important for they are an integral part of a package that will present an accurate and complete picture of you and your qualifications. Your letters of recommendations should come from professors, academic advisers, and or attending physicians who are familiar not only with your academic abilities, but with your personal interests and background.
These letters are essential in relating your ability to be successful in intern program. The committee considers not only academic achievement, but leadership ability and community service as well. Even though not required, we recommend that one letter of recommendation be from an academic source, and the other from source that can attest to your role in the community.
The Personal Statement
The personal statement presents an opportunity for you to speak about yourself. The application committee is interested learning about you, and what sparked your desire and interest in this medical specialty. Your essay should show that you can articulate your ideas clearly, logically and in an orderly fashion. Please submit your statement using standard writing procedures. (Times New Roman, 12 point, black print, with well defined paragraph structure, and grammar)
The curriculum vitae (CV) in lieu of a resume is used when applying for academic, education, scientific or research positions, fellowships or grants. Please note that there is a difference between a CV and a resume. A curriculum vitae is a longer (up to two or more pages), more detailed synopsis of your background and skills. It includes a summary of your educational and academic backgrounds as well as teaching and research experience, publications, presentations, awards, honors, affiliations, grants, fellowships, internships, and professional associations and licenses. A resume is a condensed version of a CV and does not satisfy our application requirements.
